Labiaplasty is a surgical procedure that involves removing excess labial tissue and reshaping the labia minora to achieve a more natural and aesthetically pleasing appearance of the female genitalia.

Due to the delicate nature of the labia minora, this procedure requires a high level of surgical skill. However, when performed with care, recovery is typically quick and scarring is minimal.

Why do labia minora enlarge?
The size of the labia minora is primarily influenced by genetic factors and varies significantly among individuals.
However, it’s reported that various factors can contribute to their enlargement over time.

Studies suggest that labial hypertrophy may occur due to:

These factors have been associated with changes in the size and shape of the labia minora.

What is considered abnormal?
The size, shape, and color of labia minora vary greatly among individuals.
At Swan Clinic, we’ve leveraged our experience, having treated the highest number of cases in Japan, to conduct clinical research on the morphology of Japanese women’s external genitalia, including the labia minora. (This research was presented at the 2023 Japan Society of Aesthetic Surgery (JSAS) conference.)
According to our statistical data, the average size of labia minora in Asian women is 1.4 cm, with about 30% having labia minora exceeding 2 cm. Additionally, for 60% of women, the labia minora are concealed when the legs are closed. Those with labia extending beyond this might be considered to have larger than average labia (Study sample: 722 individuals).
Our clinic is unique in conducting this type of clinical research. The findings we’ve presented at conferences are now used as a reference by doctors across Japan when performing labiaplasty procedures.

Enlarged labia minora can cause various issues, including discomfort from rubbing against underwear, itching and moisture buildup when wearing pads, as well as occasional pain. Additionally, smegma, a secretion that can accumulate between the labia majora and labia minora, may decompose and lead to unpleasant odors.

At Swan Clinic, we achieve a natural-looking result by using a V-shaped incision technique when suturing. This approach prevents the thickening at the sutured edges, ensuring a more aesthetically pleasing outcome.

At Swan Clinic, we perform labiaplasty by making a V-shaped excision at the edge of the labia minora and suturing the two sides together. This technique prevents thickening of the edges, resulting in a natural appearance as if the labia were always small.

Furthermore, we use absorbable sutures and employ an intracuticular suturing technique, which leaves no visible stitches on the surface. This method eliminates the risk of visible scarring that can occur when sutures dig into the skin.

Typical Approach at Other Clinics

In general, most clinics perform labiaplasty by making a straight cut at the edge of the labia minora. As a result, when suturing both sides together, the edges can become thick and unnatural-looking,which may result in an unattractive appearance.

Moreover, these clinics often use superficial sutures that can dig into the tissue. This may leave visible scars even after the stitches are removed. Consequently, while the overall size of the labia may be reduced, there’s a risk of an uneven or irregular final appearance.

In many hospitals, it typically takes about a week for patients to be free from post-surgical pain, with some experiencing significant discomfort, especially during the first few days after surgery.

However, patients at our clinic report remarkably low pain levels. On a scale of 0 (no pain) to 10 (extreme pain), our patients rate their pain at an average of 1.8, even on the day of surgery and the day after, which are typically considered the most painful. Additionally, the average duration of pain medication use is only 2.7 days, indicating significantly less pain than at other clinics.

Unless particularly sensitive to pain, most patients can return to work the day after surgery. While swelling typically lasts about a month at other clinics, our patients report an average of just 5 days of noticeable swelling. This rapid recovery after surgery is one of the major benefits of our approach.

    Upon arrival at our clinic, you’ll be asked to fill out a specialized questionnaire designed for gynecological plastic surgery. This allows us to gather detailed information about your concerns.

    We understand that many patients find it challenging to openly discuss sensitive issues during a face-to-face examination with a doctor. Our comprehensive questionnaire is designed to ensure that the physician can understand all your concerns, even the most minute details, without you having to verbalize them directly.

    During the examination, we conduct precise measurements of the area (including length and color assessment using a colorimeter) to thoroughly evaluate your condition. For those who wish, we offer the option of capturing images of the external genitalia using an iPad. We can then discuss the necessity of surgery and potential design options by drawing directly on these images.

    Our clinic doesn’t have a communal waiting area. Instead, you’re immediately guided to a private room upon arrival, ensuring you generally won’t encounter other patients.

    Additionally, our staff members have personal experience with gynecological plastic surgery procedures. This enables us to provide empathetic support and address any anxieties you may have before and after the surgery with genuine understanding.

    Surgical Procedure

    Specialized leg supports are used during surgery. These supports have a soft yet sturdy structure that gently cradles the legs from the knee down, minimizing strain on the lower limbs. This design further minimizes the risk of complications such as nerve damage to the lower extremities.

    During the procedure, the lower body is exposed, which can result in cooling. However, the cover portion of our supports wraps around the legs, keeping them warm throughout the surgery. Notably, this equipment is both expensive and large, typically only found in facilities specializing in gynecological plastic surgery.

    After the surgery, the physician will directly discuss the intraoperative findings with you prior to discharge. This allows you to ask any questions or voice any concerns about the procedure or recovery.

    We conduct a follow-up via LINE (a messaging app) 10 days after the surgery. It’s natural for everyone to feel anxious post-surgery, and this service allows you to easily ask questions about even minor concerns without visiting the clinic in person.

What is the typical size of the labia minora?
The size of the labia minora varies from person to person. For Asian women, as a general guideline, it’s considered average if they extend slightly beyond the labia majora.
Will the labia minora get bigger again after labiaplasty?
There’s no need to worry about that. You can rest assured it won’t happen.
How long do we need to take off work?
This procedure doesn’t put much strain on your body, so if you have a desk job or do household chores, you can typically return to work the next day. However, taking two days off would be ideal for peace of mind.
If your job involves standing for long periods or working outside, it’s best to take 2-3 days off and rest properly. This will help speed up your recovery process.
Will the surgery affect sexual sensation or cause any problems during intercourse?
While there may be some temporary numbness after the procedure, this surgery doesn’t risk damaging any nerves. As the incision heals, you’ll regain your normal sensation. There’s no need to worry about long-term loss of feeling or sexual dysfunction.
I’m worried about pain during the surgery.
At our clinic, we typically perform the procedure under intravenous anesthesia. This means you won’t feel any pain from start to finish.
Will this affect my ability to give birth?
This procedure doesn’t affect the function of the vagina, which is what’s involved in childbirth. So there’s no need to worry about it impacting future deliveries.
Can you remove the dark discoloration on the labia minora?
Yes, it’s possible to improve this. During the labiaplasty procedure, we can remove the areas with noticeable dark pigmentation.

Anesthesia

Typically intravenous sedation, so you’ll be asleep during the procedure. (May vary based on medical history.) We also provide anesthesia to manage post-operative pain.

Pain

Subsides within 3 days to 1 week after surgery.

Swelling and Scarring

Scars are minimal. Swelling and redness vary by individual but usually resolve within 1-2 weeks.

Pace of Hospital Visits

For one-day procedures (no sutures): No follow-up visit is required.

You’ll receive a 3-day oral medication (antibiotics and pain relievers) and a topical ointment.

Showering and Bathing

You can take showers starting the day after your procedure. However, please avoid baths as soaking in warm water may prolong swelling. For one-day procedures (without sutures), avoid baths for 2 weeks. For procedures with sutures, please wait 1 month before taking a bath.

Sexual Activity

For one-day procedures (without sutures), you can resume sexual activity after 2 weeks. For procedures with sutures, please wait 1 month before resuming sexual activity. (Please note that healing time, pain levels, and recovery periods can vary from person to person. These guidelines may change based on your post-operative progress.)

Post-Operative Care

On the day of your procedure, avoid long periods of walking. Once you’re home, rest as much as possible and avoid touching the treated area unnecessarily. You can typically return to regular desk work the next day, but we recommend using Exparel alongside your other pain medication for better pain management.
